amfotericine B liposomaal (Ambisome) iv 5mg/kg 1dd
Remarks:
  • Zo mogelijk tevens chirurgisch debridement.
  • Behandelingsduur 6 weken-6 maanden op geleide van de resolutie van infectie.
  • Bij aangetoonde gevoeligheid eventueel step-down naar posaconazol of isavuconazol tabletten onder TDM

posaconazol iv 300mg 1dd
Remarks:

Bij falen van of intolerantie voor amfotericine B (niet bij cerebrale lokalisatie)

 2nd choice 		Medication:

isavuconazol iv loading dose 200mg, 3dd 2 days

followed by

isavuconazol iv 200mg 1dd
Remarks: Bij falen van of intolerantie voor amfotericine B (ook bij cerebrale lokalisatie)
