cefuroxim iv 1500mg one-off
Remarks:

thoracotomie, VATS

 

Verlengde profylaxe bij:

- Sleeve Lobectomie: 5 dagen amoxicilline/clavulaanzuur 3 x dgs 500/125 mg.

- Bij plaatsen vreemd materiaal: 5 amoxicilline/clavulaanzuur 3 x dgs 500/125 mg.

- Trachea resectie: 5 amoxicilline/clavulaanzuur 3 x dgs 500/125 mg.

vancomycine iv 1g one-off

clindamycine iv 600mg one-off
Remarks:

dit advies geldt bij een sterk vermoeden op- of bewezen cefazoline-allergie of een eerdere ernstige ‘delayed type’ allergie op een ß-lactam antibioticum.

General comments

* Hartchirurgie/cardiale devices:

 

Pre-operatief: Cefazoline 2 gram iv pre-operatief <60 min voor incisie

 

Per-operatief: Cefazoline dient herhaald te worden, indien:

  • Operatie langer duurt dan 4 uur na pre-operatieve gift cefazoline, EN/OF
  • >1500 ml bloedverlies tijdens de operatie

Vervolgdosering per-operatief:

  • Indien resterende operatieduur <3 uur, dan cefazoline 1 gram
  • Indien resterende operatieduur >3 uur, dan cefazoline 2 gram

 

Post-operatief: Cefazoline iv tot maximaal 24 uur postoperatief:

  • 8 uur na laatste pre/peroperatieve gift: cefazoline 1 gram iv
  • 16 uur na laatste pre/peroperatieve gift: cefazoline 1 gram iv

Hartoperaties en implantatie pacemaker/ICD: bij bekende S. aureus-dragers wordt aangeraden voor te behandelen met mupirocine-neuszalf.

 

 

** Longchirurgie:

 

Pre-operatief:  Cefuroxim 1500 mg iv pre-operatief <60 min voor incisie

 

Per-operatief: Cefuroxim 1500 mg iv dient herhaald te worden, indien:

  • Operatie langer duurt dan 3 uur na pre-operatieve gift cefuroxim, EN/OF
  • >1500 ml bloedverlies tijdens de operatie
