General comments

In de behandeling van patiënten met een chronische pulmonale aspergillose dan wel een sub-acute invasieve aspergillose die intolerant zijn voor triazolen of geïnfecteerd met triazool resistente pathogenen, kan een echinocandine worden gegeven. 

Binnen het Radboudumc (longziekten) is afgesproken dat in deze gevallen caspofungin kan worden toegepast in een dosering van 3 maal per week 100 mg (<80kg) of 3 maal per week 140mg (>80kg). In de thuissituatie wordt deze therapie veelal gegeven via een elastomeerpomp. 

Waar gewenst kan, na consultatie van het schimmelteam, eenzelfde regime ook voor niet-longpatiënten worden aangeboden indien behandeling in de thuissituatie wordt voortgezet.
