Medication:

voriconazol po loading dose 400mg, 2dd 1 days

followed by

voriconazol po 200mg 2dd
Remarks:

Lokale behandeling combineren met systemische therapie

General comments

Lokale behandeling

  • Voriconazol druppels 10 mg/ml (1%) 3 druppels 3 dd

Voor start lokale behandeling

  • Debris verwijderen
  • Contact met apotheek opnemen voor het bestellen van voriconazol oordruppels
  • Bij holtevorming voriconazol druppels op tampon
  • Keuze van antifungale therapie zo nodig aanpassen op basis van gevoeligheidsbepaling

Behandelduur minimaal 4 weken daarna follow-up in multidisciplinair mycologie overleg (arts-microbioloog, KNO-arts, apotheker, internist-infectioloog) op basis van kliniek en beeldvorming

Spiegelbepaling verrichten bij het systemisch gebruik van voriconazol (zie dit SWAB boekje bij overig, TDM antimicrobiele middelen)
