flucloxacilline iv 6g/24h

Remarks:

Oplaaddosering 1 gram flucloxacilline bolus, daarna continu infuus

(Indien continu infuus niet mogelijk, dan flucloxacilline 6dd1 gram)

Indications: Allergie bij penicillines*

cefazoline iv 2000mg 3dd 6 weeks
Remarks:

Niet-ernstige penicilline allergie
Medication:

clindamycine po 600mg 3dd 6 weeks
Remarks:

Beta-lactamallergie

General comments

Zonder bacteriemie:

  • Totale duur antibiotica bij ongecompliceerd beloop 6 weken
  • Ten minste 1 week intraveneuze behandeling
  • Bij gebleken gevoeligheid zodra goede klinische respons switch naar orale therapie met clindamycine 600 mg 3dd oraal. Bij clindamycineresistentie zijn cotrimoxazol 960 mg 2dd oraal of flucloxacilline 1000 mg 4dd oraal een alternatief

Met bacteriemie:

  • Tenminste twee weken intraveneus behandelen en afhankelijk van andere strooihaarden langer (bijvoorbeeld endocarditis)
  • Vraag bedside consult infectioloog
