Remarks:
Indien continue infuus niet mogelijk, dan flucloxacilline 6dd1 gram. In geval van septisch ziek en/of immuunsuppressiva en/of bekende klepafwijking of kunstklep: geef flucloxacilline iv oplaaddosis 2 g, 12g continu per 24 uur ivm risico op endocarditis. Dosering verlagen naar 6 gram per 24 uur indien er geen sprake blijkt van bacteriëmie.

General comments
Rifampicine slechts toevoegen bij DAIR of 1-stage exchange. Niet bij 2-stage exchange of chronische therapie.
Rifampicine toevoegen zodra stafylokokken zijn aangetoond. Indien deze ongevoelig blijkt te zijn voor rifampicine, rifampicine weer staken.
Dosering rifampicine verlagen naar 300 mg 2dd in geval van bijwerkingen.
