General comments
Rifampicine slechts toevoegen bij DAIR of 1-stage exchange. Niet bij 2-stage exchange of chronisch suppressieve therapie
Rifampicine toevoegen zodra stafylokokken zijn aangetoond. Indien deze ongevoelig blijkt te zijn voor rifampicine, rifampicine weer staken.
Dosering rifampicine verlagen naar 300 mg 2dd in geval van bijwerkingen
