amoxicilline po 500mg 3dd 3 days
Remarks:

- Als patiënt klinisch niet verbetert en niet voldoet aan klinische stabiliteitscriteria* op dag 3, totale antibioticaduur verlengen naar 5 dagen.

- Als patiënt immuungecompromitteerd, dan behandelduur 5 dagen.

doxycycline po loading dose 200mg, 100mg 1dd 7 days

General comments

clinical stability criteria

  • Temperatuur < 37.8
  • Pols < 100
  • Ademhalingsfrequentie < 24
  • SO2 > 90 (zonder mechanische ventilatie, zonder zuurstofmasker; O2 toediening per neusbril wel toegestaan)
  • Systolische bloeddruk > 90
  • Geen tekenen van delier

AMBU-65 score

De behandeling is afhankelijk van de ernst van de pneumonie. Deze wordt gebaseerd op het aantal criteria van de AMBU-65 score, waaraan de patiënt voldoet:

AMBU-65 score:

  • Ademfrequentie > 30 per minuut
  • Mentale toestand: recente desoriëntatie in tijd, plaats of persoon
  • Bloeddruk systolisch < 90 mmHg en/of diastolisch < 60 mmHg
  • Ureum > 7 mmol/l
  • Leeftijd > 65 jaar

Een Legionellasneltest wordt < 24 uur na opname (binnen werktijd) verricht bij:

  • risicofactoren voor Legionella (recent buitenlandbezoek, tijdens een Legionella-epidemie of niet opknappen onder therapie)
  • AMBU-65 score groter of gelijk aan 2
