General comments
* risicofactoren
Roken, opgebouwde kaak (vooral maxilla), diabetes mellitus, gebruik immunosuppressiva, gebruik bisfosfonaten, gebruik van autoloog, alloplast of Xenogeen materiaal met tegelijk plaatsen van een implantaat.
Otologie: Profylaxe bij inleiding 30 mg/kg, maximaal 2 gram, bij langdurige ingreep: herhalen na 4 uur uur met 15 mg/kg, max 1 gram
Cefazoline voldoet aan alle criteria, daarom 1 keuze middel
clindamycine bij allergie tegen beta-lactam middelen. zie protocol: Beleid bij allergie voor antibiotica | Radboud UMC
amoxicilline/clavulaanzuur bij contaminatie met anaeroben
Let op: Pseudomonas, enterobacterales wordt niet gedekt met cefazoline
Bij een aantal voorschriften is er nog een kennishiaat mbt behandeling dit wordt verder uitgezocht, dit betreft:
- sanering voor chronische otitis media met/zonder cholesteatoom ZONDER actief pre- of perioperatief otorrhoe
- sluiten liquorlekkage
- verlengde profylaxe bij schedelbasis chirurgie
- verlengde profylaxe bij subtotale petrosectomie in verband met cholesteatoom
